We are looking for a dedicated and experienced Case Manager to join our team in Ajman, UAE. The ideal candidate will have a background in the pharmaceutical industry and a passion for helping patients navigate their healthcare journey.

Responsibilities:
- Serve as the primary point of contact for patients, healthcare providers, and insurance companies
- Coordinate patient care by scheduling appointments, arranging for necessary tests or procedures, and ensuring timely follow-up
- Conduct comprehensive assessments of patients' medical, social, and financial needs
- Develop and implement individualized care plans in collaboration with healthcare providers
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed
- Advocate for patients to ensure they receive the appropriate level of care and support
- Maintain accurate and up-to-date documentation of all patient interactions and interventions
- Collaborate with other members of the healthcare team to ensure continuity of care

Requirements:
- Bachelor's degree in nursing, social work, or a related field
- [X] years of experience in case management, preferably in the pharmaceutical industry
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office and electronic medical record systems
- Knowledge of healthcare laws and regulations in the UAE
